The Health and Wellness Services Department encompasses the Health Coaching program as well as a number of grant-funded initiatives and research projects focused on innovative disease management and risk reduction interventions. Our goal is to support the patient-provider relationship and plan of care, utilizing evidence-based practice guidelines and patient empowerment (self-management) strategies, such that members are encouraged and supported to actively participate in improving their health status.
Health Coaching is available to any Denver Health Medical Plan member including Medical Care, Point-of-Service, Medicare Select, Medicare Choice and CHP+. It is also available for those members with Medicaid Choice. Health coaching works best for members who want to make behavior changes for health conditions and/or for general health and wellness improvement.
The Telephonic Depression Intervention is a grant-sponsored program that offers telephonic counseling for depression to qualified patients to assist primary care providers improve the management of depression treated in the primary care setting.
